Quick mental health survey

Health is both mental and physical. 

We’ve reduced lengthy and cumbersome surveys into an efficient, mental health assessment for anxiety and depression. Leveraged from a clinically validated questionnaire, these five questions about anxiety or depression are presented in a simple survey. Machine learning algorithm then takes the responses and forms an anxiety or depression risk score alongside the results from the scan.
